How to care for your teeth and braces properly during orthodontic treatment?

Orthodontic treatment is an investment in a healthy, beautiful smile. However, to achieve the best result and avoid unpleasant complications, maintaining meticulous oral hygiene throughout the treatment period is essential.

How to care for your teeth and braces properly during orthodontic treatment?

Why is hygiene so important during orthodontic treatment?

The brackets and archwires of a fixed appliance create numerous niches where food debris and bacterial plaque accumulate.

How to brush your teeth with braces — step by step

  1. Preparation: Remove all removable elements, such as elastic bands. Rinse your mouth thoroughly with water to remove larger food particles.
  2. Brushing: Use a manual toothbrush (ideally a special orthodontic one with a V-shaped groove), an electric toothbrush with an orthodontic head, or a sonic toothbrush. Brush where the brackets are bonded, then the part of the tooth between the bracket and the incisal edge. Angle the brush head at 45 degrees to the gumline and use gentle, vibrating-sweeping motions. Thoroughly clean the tooth surfaces above and below the archwire. Don't forget the inner surfaces of your teeth.
  3. Interdental cleaning: This is a crucial and often overlooked step! Use interdental brushes (also known as single-tufted brushes) to clean the spaces under the orthodontic wire and around each bracket.
  4. Flossing: Although more difficult, it is essential. Use special dental floss with a stiffened tip (e.g. Super Floss) or standard floss with a floss threader to pass it under the wire.

A water flosser is a helpful addition — though not a substitute for floss. A pressurised stream of water effectively flushes out food debris and massages the gums, improving their circulation.

How to care for the orthodontic appliance itself?

Proper hygiene applies not only to your teeth but to the appliance itself. Your comfort and the effectiveness of treatment depend on how well you maintain it.

Care of a fixed appliance

The key is the thorough cleaning of all its components — brackets, archwires, and bands — as mentioned above. Regularly check in the mirror that no element has been damaged.

If a bracket comes off or the wire starts to poke, cover the irritating spot with orthodontic wax and contact your orthodontist promptly.

How to clean a removable orthodontic appliance?

If you wear a removable appliance, remember to clean it every time you take it out of your mouth. Use a separate toothbrush and a mild liquid soap/washing-up liquid, or special cleaning tablets.

Do not clean it with regular toothpaste, as it contains abrasive substances that can scratch its surface. After cleaning, rinse it thoroughly and store it in a dry, ventilated case.

Professional hygiene — an important part of orthodontic treatment

Even the most diligent home hygiene may not be enough to reach all the nooks and crannies created by a fixed appliance. Over time, unnoticed bacterial plaque hardens into tartar, which cannot be removed with a regular toothbrush.

1. Scaling

This is the precise removal of hard tartar that accumulates above and below the gumline, as well as around the appliance brackets.

2. Sandblasting

This involves removing deposits and stains using a stream of water with fine cleaning particles. It is extremely effective at reaching hard-to-access areas around the appliance components.

3. Polishing and fluoride treatment

Smoothing the tooth surfaces makes it harder for bacterial plaque to reattach, while applying a fluoride preparation strengthens the enamel and protects it against cavities and the formation of white spots.

Orthodontic patients are generally advised to undergo professional hygiene cleaning every 3–4 months.

Diet for the orthodontic patient — what to eat and what to avoid?

Your diet will also require some modifications. The right diet will help you avoid appliance damage and discomfort. This is especially important in terms of what you can eat after getting braces fitted, when teeth and gums are most sensitive.

In the first few days after getting braces, opt for soft foods: cream soups, yoghurts, smoothies, purées, cooked vegetables, and pasta.

What to avoid throughout the entire treatment?

  • Hard foods: Nuts, hard sweets, popcorn, hard apples (cut into pieces before eating).
  • Sticky and chewy foods: Caramels, toffees, chewing gum (can pull off brackets).
  • Hard crusts and chips: Biting can debond brackets.

FAQ — Frequently Asked Questions

1. How long should I brush my teeth with braces?

Precisely cleaning all tooth surfaces and appliance components should take between 3 and 5 minutes.

2. Can I use an electric toothbrush?

Yes, many people find that electric toothbrushes (sonic or rotary) are even more effective. Choose a model with a gentle cleaning mode and a special orthodontic head to maximise results.

3. Can I drink coffee, tea, or eat foods that stain?

It is best to limit your intake of strongly staining drinks and foods, especially if you have aesthetic (clear) ligatures on your appliance, which discolour very easily.

If you cannot do without them, always rinse your mouth thoroughly with water after eating, and brush your teeth as soon as possible.

4. Can I play sport while wearing an orthodontic appliance?

Yes, wearing braces does not exclude you from physical activity. However, extra caution is advised, especially in contact sports (e.g. martial arts, football, basketball). To protect your teeth, lips, and the appliance itself from injury, it is worth getting a special mouthguard designed for people undergoing orthodontic treatment.
